UK: Investigation into systemic failures after serial sex offender convicted

London: On 8 August, The Times reported further details of the case of Simon Levy, a convicted sex offender in London. Levy was found guilty of murdering two women and of multiple sexual offences against women. The report highlighted serious failures by police, prosecutors and probation services in managing the risk posed by him. Despite previous allegations and arrests, delays and shortcomings in the handling of his cases allowed him to remain free and commit further offences. Authorities have acknowledged the failures, and investigations into the handling of the case are continuing.  

Action to Be Taken: 
Authorities should conduct an independent review of the failures in the case and strengthen procedures for identifying and managing repeat offenders. Police, prosecutors and probation services should share information promptly, conduct proper risk assessments and respond quickly to serious allegations of violence against women. Stronger accountability and improved coordination are necessary to prevent repeat offending and protect women from further violence.
